Forms that ask for Child Custody, Visitation and Support:

  • Child Custody, Visitation, and Support Request (Form DV-105) Opens new window
  • Declaration Under Uniform Child Custody Jurisdiction and Enforcement Act
    (UCCJEA)

If you are the spouse or domestic partner of the person you want protection from, and you want to request spousal, partner, or family support:

  • Check box 10 on your Request for Order (Form DV-100).

If you are the joint parents of any children with the other person and are not married, the court may accept an agreement (called a "stipulation") to parentage at the hearing and enter a Judgment of Parentage.
